At Body Maintenance Centre, we offer physiotherapy and exercise physiology services funded through NDIS, Medicare (CDM), DVA, WorkCover, TAC, and Private Health Insurance. Below you’ll find what you need to provide, referral requirements, and how billing works for each funding option.
🧾 NDIS (National Disability Insurance Scheme)

🏥 Medicare CDM (Chronic Disease Management Plan)
📋 What We Need

A valid CDM referral from your GP specifying physiotherapy or exercise physiology

Information on how many allied health visits you have left this year (max 5 visits per calendar year)

🔁 Referral Requirements

The referral must be signed and dated by your GP

💳 Billing Information

Medicare rebate is $58.30 per session (2025 rate)

Bulk billing may be available for eligible clients

🎖️ DVA (Department of Veterans’ Affairs)
📋 What We Need

A DVA referral from your GP (D904 form or letter) specifying physiotherapy or exercise physiology

Details of your accepted condition if you hold a White Card

⚙️ WorkCover & TAC (Injury Rehabilitation)
📋 What We Need

Claim number and insurer or case manager contact details
