By completing the Informed Consent form and signing below, you acknowledge and agree to the following:

Personal Belongings: Body Maintenance Centre is not responsible for any lost, stolen, or damaged valuables, cash, or personal items.

Compliance: You agree to follow all rules and directives set by Body Maintenance Centre and any facility (“Facility”) where our services are provided.

Voluntary Participation: You have voluntarily engaged the services of Body Maintenance Centre and are using our services and the Facility at your own free will.

Medical Information: You have fully disclosed all relevant information regarding your medical history, current health status, and medical conditions. To the best of your knowledge, all information provided is complete and accurate.

Media Consent: You grant Body Maintenance Centre and its affiliates permission to use photographs and video footage of you for marketing purposes, including publicity, print, web content, and online advertising.

Scope of Consent: This consent extends to all services provided, including hydrotherapy and manual therapy, and applies to any associated Facility such as pools used for hydrotherapy.

Payment Authorisation: If you have provided credit card details, you authorise Body Maintenance Centre to store and use these details through its practice management software (currently Halaxy & Trainerize) for payment of future invoices. You acknowledge that Body Maintenance Centre is not responsible for the storage or processing of your payment details beyond entering them into the software, and you accept the payment processing terms of these services, which can be viewed at Halaxy Terms.

Acknowledgement of Risk: You understand that participation in our services and use of the Facility involves inherent risks, including physical and non-physical harm. You accept these risks, which may include injury, property damage, or death.

Exercise Risks: You acknowledge that exercise and physical activity are physically demanding and may pose risks to your health, which you accept.

Release and Indemnity: You release and indemnify Body Maintenance Centre, its contractors, employees, directors, affiliates, and the Facility from any claims, damages, or liabilities arising from your use of the services or Facility, whether caused by negligence, accident, or other causes.
Duty of Care & Treatment Risks
As part of our duty of care, we inform you of the potential risks associated with professional physiotherapy treatments such as massage, myofascial release, traction, and joint mobilisations. While these techniques are generally safe, there is a small risk of injury to nerves, bones, muscles, ligaments, discs, or arteries.
Other possible risks include:

Allergic reactions to massage oils, strapping tape, or topical applications

Minor bruising, particularly if you are taking certain medications or have specific health conditions
You will receive a verbal explanation of your examination results and proposed treatment techniques before consent is sought. You have the right to refuse any treatment offered and seek a second opinion at any time.
You also consent to your therapist sharing relevant information with your doctor, specialists, or trainers, except where you have specifically requested confidentiality.
Infectious Diseases
You are responsible for informing your therapist if you have any infectious diseases (e.g., TB, HIV, Hepatitis, Glandular Fever, Golden Staph), as some manual therapy techniques may increase the risk of transmission.
Medicare & Appointment Policies

For Medicare Chronic Disease Management Plan bulk-billed sessions, if a claim is denied by Medicare, you agree to pay any outstanding balance directly to Body Maintenance Centre.

Please provide at least 24 hours’ notice for cancellations or rescheduling. Failure to do so may result in a cancellation fee, depending on the circumstances.
