Referral Information

We accept all Medicare, DVA and Work Cover referrals

BULK BILLING AVAILABLE FOR PENSIONERS OR CONCESSION CARD HOLDERS

OR UPON SPECIFIC GP REQUEST

Medicare: For us to bulk bill your session we need to have a copy of your formally known Enhanced Primary Care (EPC) plan or your Chronic Disease Management (CDM) plan, signed by your GP. The sheet will have your GP provider details, your date of birth, your Medicare details and the number of sessions you have been allocated, including the date of the referral.

DVA: For us to bill your session we need a copy of your D904 paperwork, signed by your GP.

Work Cover/TAC: For us to bill your session we need a referral from your GP, your work capacity and/or medical certificate - worker's compensation, your claim number and Case Manager details.
